{"id":961,"date":"2016-11-13T13:58:15","date_gmt":"2016-11-13T12:58:15","guid":{"rendered":"http:\/\/demos.codexcoder.com\/topnews\/?p=961"},"modified":"2018-05-07T16:01:39","modified_gmt":"2018-05-07T14:01:39","slug":"le-projet-2","status":"publish","type":"post","link":"http:\/\/apajhetvous.apajh.org\/SIAM92\/le-projet-2\/","title":{"rendered":"Le projet"},"content":{"rendered":"<p>L\u2019intervention du SIAM 92 rel\u00e8ve d\u2019une orientation prononc\u00e9e par la Maison D\u00e9partementales des Personnes Handicap\u00e9es (MDPH) et concerne une situation de handicap visuel, de la malvoyance \u00e0 la c\u00e9cit\u00e9 ; Elle se situe secondairement au diagnostic m\u00e9dical de d\u00e9ficience visuelle pos\u00e9 par le m\u00e9decin.<\/p>\n<p>Le service n\u2019\u00e9tant pas un centre de consultation m\u00e9dicale, les soins m\u00e9dicaux demeurent exerc\u00e9s par le m\u00e9decin traitant choisi par la personne.<\/p>\n<p>Les prestations du SIAM 92 sont d\u2019ordres th\u00e9rapeutique, \u00e9ducatif, p\u00e9dagogique et social. Elles sont personnalis\u00e9es selon un projet personnalis\u00e9 d\u2019accompagnement (PPA) \u00e9tabli en concertation avec la personne et sa famille et men\u00e9es sur ses lieux de vie (domicile, structure d\u2019accueil, trajets\u2026).<\/p>\n<p>Plusieurs actions sont mises en place par le SIAM<\/p>\n<ul>\n<li>\u00c9laborer, mettre en \u0153uvre et ajuster le projet personnalis\u00e9 d\u2019accompagnement en concertation avec le jeune et sa famille.<\/li>\n<li>Participer au projet personnalis\u00e9 de scolarisation du jeune, scolaris\u00e9 en milieu ordinaire au plus pr\u00e8s de son domicile ou dans un dispositif sp\u00e9cialis\u00e9 de l\u2019\u00c9duca-tion nationale.<\/li>\n<li>Apporter les aides palliatives au handicap visuel : humaines, techniques, mat\u00e9rielles.<\/li>\n<li>Participer aux adaptations des supports (p\u00e9dagogiques, de culture, de loisirs) et \u00e0 l\u2019am\u00e9nagement de l\u2019environnement en diverses situations de vie (familiale, scolaire, sociale\u2026).<\/li>\n<li>Mettre \u00e0 la disposition du jeune, de sa famille et des acteurs de son environne-ment des connaissances et comp\u00e9tences sp\u00e9cifiques relatives aux cons\u00e9quences fonctionnelles et situationnelles de son handicap visuel.<\/li>\n<li>D\u00e9velopper des partenariats et des r\u00e9seaux, d\u00e9ployer l\u2019information et la communication, aux fins de favoriser l\u2019\u00e9panouissement de la personne d\u00e9ficiente visuelle et sa participation \u00e0 toute forme de vie sociale.<\/li>\n<\/ul>\n<p>Ces actions ont pour vocation la r\u00e9alisation de divers objectifs\u00a0:<\/p>\n<ul>\n<li>Pr\u00e9venir et limiter l\u2019\u00e9mergence et l\u2019installation des processus d\u2019inadaptation cons\u00e9cutifs \u00e0 la malvoyance et \u00e0 la c\u00e9cit\u00e9 d\u00e8s la naissance.<\/li>\n<li>Soutenir l\u2019acc\u00e8s et l\u2019accueil du jeune d\u00e9ficient visuel aux espaces communs d\u2019\u00e9ducation, de scolarisation, de formation, de culture et de loisirs.<\/li>\n<li>D\u00e9velopper l\u2019autonomie dans toutes les circonstances de la vie en menant un accompagnement adapt\u00e9 aux besoins, potentialit\u00e9s et d\u00e9sirs du jeune.<\/li>\n<li>Accompagner, soutenir et informer les jeunes en situation de handicap visuel, leur famille et les professionnels des structures<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>L\u2019intervention du SIAM 92 rel\u00e8ve d\u2019une orientation prononc\u00e9e par la Maison D\u00e9partementales des Personnes Handicap\u00e9es (MDPH) et concerne une situation de handicap visuel, de la malvoyance \u00e0 la c\u00e9cit\u00e9 ; Elle se situe secondairement au diagnostic m\u00e9dical de d\u00e9ficience visuelle pos\u00e9 par le m\u00e9decin.
